Patient demographics vary as we strive to provide services for all eligible patients. We serve the entire Los Angeles area mainly the San Gabriel and San Fernando Valleys. We look to expand our horizons and soon provide services in the greater southern California area.

Our care coordinator evaluates the eligibility of the patient. If suitable, the patient must obtain an order from their main physician or specialist for in-home
services.

With a plan of service in place by your physician our offices will coordinate the prompt and proper care necessary.

Our Team
Our dedicated on-field staff is composed of RNs, LVNs, and CHHAs who are hospice and palliative care certified. Their focus is to make this difficult time as comfortable as possible for both the patient and their loved ones.
Communication
Our in office as well as on the field staff speak not only English, but also, Armenian, Russian, Spanish, Farsi, and French.
Our Services
Our services include comprehensive pain and symptom management, focused on optimizing patient comfort and quality of life. We provide coordinated, interdisciplinary care to support terminally ill patients in the home setting, ensuring alignment with the established plan of care and timely response to clinical needs. In addition, we offer grief counseling and bereavement support services for adults, children, and families, promoting continuity of care and emotional support throughout and beyond the hospice experience.
